Now we offer convenient online forms to register for Baptism, First Communion, and Confirmation. This article provides quick links to all of the forms.
After submitting your paperwork for Baptism, please also send a copy of the birth certificate. If we do not receive the birth certificate, we will not be able to register you.
For First Communion and Confirmation, you must submit both a birth certificate and a baptism certificate. If we do not receive these documents, we will not be able to complete your registration.
